Conroe neurologist surrenders medical license
CONROE, Texas -- The Texas Medical Board has accepted the voluntary surrender of Dr. Stephen Frank Waller’s medical license, based on a November 30 agreement. Under the terms of the agreement, Waller has waived any further board or court hearings.
A neurologist, Waller’s license was suspended in May 2010 after failing to complete 20 hours of continuing medical education in medical record keeping, and for failing to pass the medical jurisprudence exam required by the Texas Medical Board. After the suspension, Waller continued to write prescriptions for controlled substances. State investigations revealed that he wrote prescriptions between May 12, 2010, and February 11, 2011.
Waller is also currently undergoing investigation by the Montgomery county District Attorney’s Office.
